Do Your Know Your NHS Number?
In order to improve patient safety and ensure patients are identified correctly whilst using NHS services, patients are advised to be aware of their own individual NHS number which is contained on your medical record card.
If you do not know your NHS number please contact the Patient Services Dept at the PCT on 0161 304 5807/813 who will be happy to provide you with this information.

If so please contact our dedicated team of Patient Services Officers on 0161-304-5807/13 or print out and fill in the GP Allocation Form. Please post it to the address on the bottom of the form.
